Welcome to Florida International University’s incident reporting website. This website guides you in sharing important information with University offices tasked with addressing and responding to student concerns, student and academic misconduct, discrimination, harassment, sexual harassment and sexual misconduct in the FIU community.
Office of Title IX and Equal Employment Opportunity Compliance is responsible for ensuring that FIU maintains a workplace and learning environment free from discrimination, where current and prospective faculty, staff, and students are treated fairly.
If any student, employee or applicant has a sincere and reasonable belief that they have been discriminated against or harassed based on age, color, disability, marital status, ethnic or national origin, race, religion, retaliation, sex or any other protected category, the University encourages them to use the resources below to report the incident.
Where to Report Your Concern
Title IX: Sexual Harassment or Misconduct
Use this form to report any possible violations of FIU's Sexual Harassment (Title IX) and Sexual Misconduct Regulation, such as any form of non-consensual sexual activity and unwelcome sexual conduct including: sexual assault (rape/sexual battery), sexual coercion, sexual harassment, dating violence, domestic violence, non-consensual sexual touching, stalking, obscene or indecent behavior, revenge porn, harassment or violence based on sexual orientation, and retaliation. Discrimination, Harassment, and Retaliation
Use this form to report any possible violations of Florida International University's Nondiscrimination, Harassment, and Retaliation Regulation such as discrimination or harassment based on race, color, sex, pregnancy, religion, age, disability, national origin, marital status, and veteran status. You can also call the Office of Title IX and EEO Compliance at 305-348-2785 if you have questions prior to submitting a report.
FIU Hazing Reporting Form
Use this form to report alleged hazing involving students, faculty, staff, or visitors, volunteers, or third parties affiliated with the University. Hazing is a violation of university policies and state law, as well as a reportable Clery offense. It includes but is not limited to, acts of coercion, forced consumption of substances, physical brutality, mental or emotional stress such as forced exclusion, sleep deprivation, public humiliation or any other conduct that would adversely affect the dignity, well-being, or lawful rights of another person.
Student of Concern
Some students will face obstacles along the path toward their academic goals, such as mental health concerns, financial hardships, housing insecurity, medical emergencies, academic challenges, and more. Through the Panthers Care initiative, the Dean of Students office can assist students one-on-one by identifying the appropriate campus resources to help get them back on track toward their goals. Use this form to report any concerning behaviors or incidents.
Student Conduct or Academic Violations
Use this form to report alleged violations of the Student Conduct and Honor Code, such as academic, behavioral, and/or organization misconduct. Reports are submitted to the Office of Student Conduct and Academic Integrity (SCAI) for review. Silent Witness Report to Law Enforcement
Use this form to anonymously notify the Florida International University Police Department (FIUPD) of incidents that you feel warrant a police response but are not critical in nature. The FIUPD relies on our community partners to inform us of incidents that require the FIUPDs attention. This includes but is not limited to crimes that have already occurred, unusual occurrences, or unsafe conditions. Please note, Silent Witness is not a means to communicate with FIUPD for emergencies. FIU Police Department Employee Misconduct
Use this form to report Florida International University Police Department (FIUPD) employee misconduct. The FIUPD internal affairs investigative process is meant to ensure that department policy and procedures are followed and that all department employees follow agency standards of professionalism. To ensure our community’s trust, maintain FIUPDs integrity, and provide a mechanism where officers falsely accused are vindicated, it is important that FIUPD investigate all allegations of police misconduct immediately, thoroughly, and objectively. All complaints shall be investigated fully, in a fair and impartial manner. If you have an emergency dial 911 or call FIUPD at 305-348-5911.
Health & Safety Reporting Form
Use this form to report hazards, near misses, safety concerns, and issues that could pose a threat to the health and safety of our students, faculty, and staff. You can submit this form anonymously; however, your personal information will be helpful in follow-up investigations. You can also email the Department of Environmental Health & Safety at ehs@fiu.edu if you have any questions prior to submitting the report.
NCAA Compliance Reporting Form
Use this form to report any violation of NCAA rules, such as any action that provides an impermissible benefit and/or recruiting, competitive, or other advantage to a student-athlete, coach, team or institution. An impermissible benefit is any special arrangement provided to a student-athlete or their family or friends not expressly authorized by NCAA legislation.
Fraud, Waste, Abuse, Financial Mismanagement, and Whistleblower Complaints Reporting Form
Use this form to report known or reasonably based suspected incidents of fraud, waste, abuse, or financial mismanagement, as well as whistleblower complaints. Reports are submitted directly to the Office of Internal Audit (OIA) for review, and in the case of whistleblower complaints, for whistleblower protection determination. Individuals reporting non-whistleblower complaints may report anonymously. However, providing your contact information will enable OIA staff to follow up with you for clarification or further information. HWCOM Learning Environment, Professionalism & Mistreatment Reporting Form
Mistreatment occurs when behavior shows disrespect for the dignity of others and unreasonably interferes with the learning process. HWCOM students should use this form to report learning environment, professionalism or mistreatment concerns not related to discrimination or harassment (sexual harassment concerns should be reported here, and all other discrimination and harassment concerns should be reported here). Students who report mistreatment are protected from all forms of retaliation.
HWCOM Graduate Medical Education: Resident/Fellow Incident Reporting Form
This reporting form provides a safe and confidential way for residents and fellows, to share concerns related to professionalism, mistreatment, or unprofessional behavior.
Your feedback is vital. It helps us identify issues early, address them appropriately, and support a healthy learning and working environment. If you have experienced or witnessed conduct that you believe warrants attention, we encourage you to use this form. All submissions will be routed to the appropriate individuals for review and follow-up.
Ethical Panther Hotline
At FIU, we believe that our ethics, integrity and commitment to compliance is essential in guiding us to make the right decisions. The FIU Ethical Panther reporting system is available 24-hours a day, seven days a week to empower all of our University Community members to exercise personal responsibility and accountability for upholding FIU’s core values. A crucial part of that responsibility is speaking up when you witness or suspect ethical violations, compliance concerns and misconduct and sharing your concerns.
