Welcome to Florida International University’s new incident reporting website. This website guides you in sharing important information with university offices tasked with addressing and responding to student concerns, student and academic misconduct, discrimination, harassment, sexual harassment and sexual misconduct in the FIU community.

If any student, employee or applicant has a good faith belief that they have been discriminated against or harassed based on age, color, disability, gender, marital status, ethnic or national origin, race, religion, retaliation, sex or any other protected category, the Office of Civil Rights Compliance and Accessibility (CRCA) encourages them to use the resources below to report the incident.

Where to Report Your Concern

  • Title IX: Sexual Harassment or Misconduct

    Use this form to report any possible violations of FIU's Sexual Harassment (Title IX) and Sexual Misconduct Regulation, such as any form of non-consensual sexual activity and unwelcome sexual conduct including: sexual assault (rape/sexual battery), sexual coercion, sexual harassment, dating violence, domestic violence, non-consensual sexual touching, stalking, obscene or indecent behavior, revenge porn, harassment or violence based on sexual orientation, gender identity or gender expression, and retaliation.   • Discrimination or Harassment

    Use this form to report any possible violations of Florida International University's Nondiscrimination, Harassment, and Retaliation Regulation such as discrimination or harassment based on race, color, sex, pregnancy, religion, age, disability, national origin, marital status, and veteran status. You can also call the Office of Civil Rights Compliance and Accessibility at 305-348-2785 if you have questions prior to submitting a report.

  • Student of Concern

    Use this form to refer a student who is exhibiting academic, emotional, or physical behaviors indicating that they may be in distress or crisis, whether in the classroom, on campus, or off campus. Examples might include consistent or unexplained absences, serious medical incidences or circumstances, basic needs insecurity (i.e., housing, food, etc.), yelling or aggressive comments, or a sudden or deteriorating change in physical appearance, emotional state, or personal hygiene. You may also use this form to self-report, and the option to remain anonymous is available. You can also call the Dean of Students Office at 305-348-3396 if you have questions prior to submitting this referral form.

  • Student Conduct or Academic Violations

    Use this form to report student conduct concerns or academic misconduct to the Office of Student Conduct and Academic Integrity.

  • Silent Witness Report to Law Enforcement

    Use this form to anonymously notify the Florida International University Police Department (FIUPD) of incidents that you feel warrant a police response but are not critical in nature. The FIUPD relies on our community partners to inform us of incidents that require the FIUPDs attention. This includes but is not limited to crimes that have already occurred, unusual occurrences, or unsafe conditions. Please note, Silent Witness is not a means to communicate with FIUPD for emergencies.   • FIU Police Department Employee Misconduct

    Use this form to report Florida International University Police Department (FIUPD) employee misconduct. The FIUPD internal affairs investigative process is meant to ensure that department policy and procedures are followed and that all department employees follow agency standards of professionalism. To ensure our community’s trust, maintain FIUPDs integrity, and provide a mechanism where officers falsely accused are vindicated, it is important that FIUPD investigate all allegations of police misconduct immediately, thoroughly, and objectively. All complaints shall be investigated fully, in a fair and impartial manner. If you have an emergency dial 911 or call FIUPD at 305-348-5911.

  • Health & Safety Reporting Form

    Use this form to report hazards, near misses, safety concerns, and issues that could pose a threat to the health and safety of our students, faculty, and staff. You can submit this form anonymously; however, your personal information will be helpful in follow-up investigations. You can also email the Department of Environmental Health & Safety at ehs@fiu.edu if you have any questions prior to submitting the report.

Retaliation, including retaliating against an individual reporting sexual harassment, sexual misconduct and other forms of discrimination or harassment, is expressly prohibited by Regulation 105 and 106.
